The property that sustained damage is located in an area designated as a disaster by the President of the United States or the Governor and I am the owner of the property or the owner's legal representative. Insurance companies will notify me directly only if they locate an insurance policy for the subject property and they have reason to believe that I am the owner or the owner's legal representative. I hereby agree to be contacted by an insurer that has located an insurance policy for the subject property. The insurance company may also require additional information from me. The role of the California Department of Insurance is to make this request available for insurers to research their database. I have read and I understand the information contained in the "Privacy Notice."
